The muscle and the place

Soleus, distal third, medial border of the Achilles tendon

sourced

What the finger seeks

Person kneeling on a chair seat, or side-lying with the knee bent, which slackens the gastrocnemius; the pincer searches below the bulge marking the end of the gastrocnemius fibres, in front of the Achilles tendon, on both sides at once.

The cord is taken between thumb and fingers on either side of the muscle in front of the Achilles tendon, then rolled; the tenderness of this place lies beneath the tendon's aponeurosis, and a pincer grip that takes only one side of the tendon misses it.

What pressure brings out

Pressure wakes the heel pain the person recognises as their own — back and sole of the heel, lower end of the Achilles tendon.

The pain gathers at the back and sole of the heel and at the lower end of the Achilles tendon.

It sometimes reaches the region of the sacroiliac joint on the same side — mostly from a rarer spot, a little higher and further out (see the variant).

A rare spot in the same distal third — slightly more proximal and lateral, not to be confused with the proximal locus on the lateral side of the calf — sends a deep pain into the sacroiliac joint of the same side, sometimes with a slight spillover to the heel; the authors report twice an exceptional pattern to the face and jaw of the same side.

The technique

Invasive technique not publishable: the structured contract is incomplete.

Caution

The authors publish no depth for this place: they give needle lengths, not a distance to a structure; the critical structure they name — the posterior tibial bundle — belongs to the deep midline, away from the medial approach with counter-pressure.

Convergence

This section requires a distance explicitly referencing this geometrically placed locus. A distance to its carrying muscle is never assigned to it.
